Application Form OHPA Awards

Participation is exclusive and limited to individuals or entities operating in Portugal.

Required Fields: Complete all sections marked as “Required” in full.
Word Limits: Adhere to the specified word limits to ensure your application is assessed fairly.
Support Documentation: Provide concrete and detailed evidence using the attachments, however, avoid including unnecessary information.
Submission: Submit applications by the deadline announced on the One Health Pet Awards official website.
Multiple Categories: Each participant may apply in more than one category but must submit a separate application for each.
Document Submission: All documents (required and additional) must be sent via WeTransfer. Include the transfer link in the designated section of the form.
